Memory Joggers: ACLS Stable Wide Complex Tachycardia

Stable = no serious signs and symptoms

Reviewed 8-2004
  • Assess ABCs
  • Secure airway
  • Administer oxygen
  • Start IV Attach monitor, pulse oximeter, and automatic B/P cuff
  • Assess vital signs
  • Review history
  • Perform problem focused physical examination
  • Order 12-lead ECG
  • Amiodarone 150mg IVP over 10-20min. (May repeat 150mg IVP every 10 minutes). After bolus, start a slow infusion of 1mg/min for 6 hours then decrease to 0.5mg/min for 18 hours. Max cummulative dose of 2.2gm in 24 hours.
  • Consider cardioversion if not converted with meds.
  • Procainamide 20-30mg/min, maximum total 17 mg/kg for uncertain wide complex tachy or VT
  • Adenosine and Verapamil are contraindicated for wide complex tachycardias.
