Packed Red Blood Cells
- Hgb < 8 and Hct less than 24
- Hgb < 9 in patient undergoing surgery or postop within 3 days with continued blood loss with active bleeding
- > 15% blood loss (750cc)
- Symptomatic anemia (hypovolemia and/or hypoxia) with Hbg < 10.0
- Anemia with Hgb < 10.0 and risk factor for ischemia (CAD, cardiac valvular disease, CHF, cerebral ischemic disease)
- Hgb < 11.0 and diagnosis of unstable angina or acute MI, age >=65
Autologous Transfusions
- Hemoglobin < 10
Platelets
- Platelet count < 20,000
- Platelet count < 70,000 in patient
- undergoing major surgery
- with active bleeding
- with severe trauma
Automatic review if diagnosis is Immune Thrombocytopenia(ITP) or Immune Thrombocytopenia Purpura
Fresh Frozen Plasma
- Thrombotic Thrombocytopenia Purpura
- PT > 16 and/or PTT greater than 40 seconds
Automatic review with no prior PT or PTT (except for TTP)
Cryoprecipitate
- Fibrinogen < 100 mg per mg/dl with unexpected clinical bleeding in the absence of thrombocytopenia or surgical prophylaxis
- vonWillebrand's diease with clinical bleeding or for surgical prophylaxis
- Hemophilia A
- Factor XIII deficiency
- Acquired severe coagulopathy (fibrinogen < 100 mg/dl) and clinical diagnosis of dissecting aneurysm, post-partum fibrinolysis, disseminated intravascular coagulation or severe end-stage liver disease
- Used as fibrin glue to aid in surgical hemostasis
