Turn on the computer
Log onto the network
Network Login = qresidents
Network Password = password
Look for the following icon (It could be located anywhere on the computer desktop and could be named various names.) Double Click on the icon

The following screen withappear:
At this screen type “PSD7”
The following screen with appear:

At this screen press “F22” (If your keyboard does not have a F22 – you would use the “shift F10”
The following screen will appear:

Physician Sign-on: FCMQ
Physician Password: FCMQ01
Hit "Enter"
After you enter your sign-on and password the following screen will appear:

To look for a medical record number type “03” in the enter field located to the right of the current screen.

You now need to enter the patient information that you have available.
Capabilities associated with this pathway are patient inquiry for medical record number, address, and phone numbers.
The “*” serves as a wild card in SMS. The asterisk must be used when trying to locate patient information. If you do not use the asterisk – the computer may give you an error message because it cannot find an exact match.
Here is an example of an inquiry for the patient “Donald McCleary”
Last Name: McCleary
First Name: Do*
Note: f the computer finds more than one match – you will need to pick the patient you would like from a list. The computer automatically lists all female patients before it lists male patients. You can limit your search by providing the gender of the patient, birthdate, and/or social security number.

Hit Enter
The following screen will appear:

The medical record number is located at the upper left of this screen. To look up additional medical record numbers “F15” (Shift F3”) – This will return you to the “Patient Inquiry Menu”.
Now you need to know how to locate a chart in medical records!!
How to locate a chart in medical records
A medical record number consists of 6 digits.
You will need to identify the 3rd, 5th, and 6th digit of the medical record (Notice that these digits are color coded).
Example Chart Number – 5_-75-39 (Quincy location has been assigned the 5_ series of medical record numbers – You will encounter 50 through 53 as the 1st and 2nd digits of a medical record number if a patient has been seen at other SIU facilities their chart number will begin with something other than 5_)
Charts are filed in numerical order beginning with the 3rd digit, which means all charts with a “0” for the third number will be filed together – then all charts with a 3rd digit of “1” will be filed together etc. Charts are filed in numerical order from 0 – 9 beginning in the stack located next to the medical record desk. The third digit can also be located through a color code system. Color codes are posted on the medical record stack.
The last two digits of a medical record number is a numerical number from 00 to 99. Once you have located the “7” the easiest way to locate the chart is now by the 5th and 6th digit. Charts are filed numerically from 00 to 99 and filed in the medical records stack from left to right from top to bottom. Once you have located the 5th and 6th digit of the medical record – Charts are then filed numerically according to the 3rd and 4th numbers. An example: 5_-75-39 would be located in “39” between charts number 5_-74-39 and 5_- 76-39.
Once the chart has been located and you need to remove it from the medical record stacks – please write on a piece of paper – the patient name – medical record number – date – your name and place the paper in an out guide where the chart was located.
