Residents Manual
Clinic and Assorted Information
Clinic
Clinic starts promptly at 9:00 a.m. and 1:30 p.m. Monday through Friday except scheduled holidays. As a first year, you will initially see 3-4 patients per clinic. All patients must be discussed with the clinic supervisor. You will gradually see more patients as you progress through your first year. Second or third year residents may see anywhere from 8-12 patients per clinic. All Medicare and Medicaid patients must be discussed with the clinic supervisor and appropriately documented. All other patients are to be briefly discussed at the end of clinic. (See Supervision Policy)
If at any time you are uncomfortable, ask the supervisor to see the patient with you. All procedures must be supervised during your first year and for all Medicare and Medicaid patients regardless of tenure. You must read and sign your dictations regularly. Attempt to dictate after each patient. (See medical records policy). Dictate and sign your charts promptly to avoid any delay in communication. Sign charts at least weekly.
You have three areas to check on a daily basis:
- Mailbox – This is for consult note, outside labs, QFPC patient discharge summaries, etc.
- Lab Shelf – Address results by telephone or mail and document your contact and follow-up plan in the chart progress note section
- Message Box – This will be near your team nurse and include charts requested, messages from patients, nursing homes, etc. Please address these messages promptly.
Please let your nurse know who will be covering you when you are away.
East Adams County Rural Health Clinic (Golden Clinic )
Golden Clinic patient care sessions begin at 9:00 a.m. and 2:00 p.m.
Hospital
Keep up with your charts. History and physicals must be dictated within 24 hours. The discharge summary should be dictated at time of discharge if possible – must be dictated within fourteen days.
Noon Conferences
Most are at the clinic, some are at the hospital. Either way, be on time, especially the days we do not have lunch. You will be responsible for various conferences throughout the year, such as, OB, Pediatric, Case Conferences, etc. Please check schedule ahead of time and be prepared. Other conferences include early bird rounds, adult rounds, etc. -- information for these is posted in the physician’s lounge. Director/Chief resident Conference monthly and resident issues, etc. (See Conferences)
Outside Activities
Be involved. Ideas for social activities and retreats are welcome.
Rotations
Ask senior residents about the specific rotation you are starting. Contact attending and find out when and where to meet, as well as what is expected of you. Review the goals/objectives/rotation mechanics and resource list on the www.quincyfp.org website.
Documentation
Document procedures in New Innovations (www.newinnov.com) as soon as you perform them.
Keep all hospital dictations for future reference.
Medical Records Policy
This policy only pertains to Quincy Family Practice Center as Blessing Hospital has its own Medical Record Policy. (See QFPC Medical Records Policy)
Delinquent Charts
Delinquent charts will be defined as charts that do not have dictation for a patient visit within 48 hours of that visit.
